When to use sic

I'd like some clarification on when to use [sic]. For example, I have this citation. The record shows the bride's surname as Denison, but her surname is actually Denson. Would it be better to use [sic] in this case or the correct surname, also in square brackets?

National Register of Historic Places nomination forms

I have recently found the nomination forms for the National Register of Historic Places have been digitized and are on NARA's website (They are also on a site from the National Parks Service, but I find the NARA citation to be more straightforward), but I'm still unsure about this citation.

Website Citation Question

Hello, 

 

I am struggling with how exactly to cite this website.

It is a Rockingham County website, but it appears to have a copyright by Tyler Technologies.  Is Tyler the owner of the site or Rockingham County?

When I search for the document, I can see a copy of the original marriage license, but I know that the physical records are kept in the Deed Room at the Rockingham County Circuit Court Clerks Office.

I'm not sure why I'm struggling with the citation for this one so much.

State hospital records

I received a patient summary record for a 2x-great-grandfather's admission to a state hospital, and I've begun to craft the citation:

H. H. Wilkerson patient summary record, case no. 17459, Rusk State Hospital, Rusk, Texas, 14 November 1950; privately held by mbcross.

I left the creator block blank because it would have introduced repetition. Because I received this document via email, would you suggest including a layer for that?
