How to cite an original baptism record

I have an original baptism record (written at the time of her baptism) from my grandmother passed down to me.  How do I cite this? I would think this is an original source created at the time she was baptized.

Iminerd13, from your description it appears you are referencing a family artifact--the personal certificate given to the family at the time the baptismal occurred--rather than the officially registered document. If so, then see EE 3.25.
