Transcribed BDM/Vital Records

I have copies of BDM's that have been obtained by a genealogical transcription service.  They provide the content of the vital record on a type sheet.  

I am seeking assistance to identify which Vital record reference from EE I should apply.

Nick, if we understand you correctly, you don't have an image of the actual document. You only have a transcript sent by a company. Ergo, you're not citing a vital record. You're citing a report from a research company.

Look in the "Business" chapter (Chapter 4). The QuickCheck Model at p. 172 or one of the report models at 4.28 would provide a base upon which to do your tweaking, to identify the exact nature of what the company sent you.
